I agree and understand that swimming is a HAZARDOUS activity. I recognize that there are risks inherent in the sport of swimming, including but not limited to, paralyzing injuries and death.The participant of AquaMobile Aquatic Programs agrees to indemnify and hold harmless AquaMobile Inc. and its coaches, officers, directors, agents and employees against any liability resulting from any injury that may occur to the participant while participating in AquaMobile Aquatic Programs. The participant also agrees to indemnify AquaMobile Inc. for any damages incurred arising from any claims, demand, action or cause of action by the participant.The participant authorizes any representative of AquaMobile Inc. to have the participant treated in any medical emergency during their participation in AquaMobile Aquatic Programs. Further, the participant and/or parent/guardian agrees to pay all costs associated with medical care and transportation for the participant.I have informed AquaMobile Inc. via email to [email protected] of any medical/health problems of which the staff should be aware.
Policies & Procedures
Cancellation Policy: No refunds or credits will be given for missed lessons that are a part of our group swim lesson programs.Inclement Weather: During periods of light rain or showers, classes will continue. However, if there is any sign of lightning or thunder, the instructor will end the lesson and a make-up lesson will be scheduled for that class.Supervision: A parent or guardian of each student must be at the lesson location if the student is a minor. The instructor reserves the right to end the lesson for that student if no guardian is present and the lesson will be charged to the client.Pool Care: In order for our instructors to provide the best learning experience possible, please ensure that your pool is kept at a minimum 84F degrees and that your pool is algae-free. The instructor reserves the right to end the lesson if pool care falls below standards.I have carefully read the above liability release and accept it with full knowledge of its contents and significance.
